If your pet is currently being seen at Urban Animal, skip this step.

SUBMIT YOUR PET’S BEHAVIOR HISTORY

SUBMIT VIDEOS OF YOUR PET IN MOTION

Collect and submit videos of your pet in motion using a unique Google Drive, Dropbox Link or by sending URL links for unlisted YouTube videos to happyanimal@urbananimalnw.com.

Follow the link below for guidance on collecting good quality videos in order to get a good quality assessment. Submit videos whether you suspect pain is present or not.
